To use YKFlex you must first submit an application. The application must be signed by a health care professional. The application form includes the following information:
- Identification of the user
- Type of mobility aid used
- If an attendant is required
- If the disability is temporary or permanent

If any information on the application changes, please inform the NWT Disabilities Council or the City of Yellowknife as soon as possible. It is important that transit staff have up to date information for each registered user in order for this service to run smoothly.
